Yes, if you've ever had a name change you must bring all the official changes, marriage certificate, and or birth certificate or passport... Their website lists everything and I highly recommend having an appointment.
Bring it If it has a star real ID then maybe it dont need to show proof of address But bring at least 2 forms of address in case. Also. Do you have another form of ID if you have no real ID star?
