The luggage carts are free to use and I went with my mother who needs assistance, the assisted pass they assign someone who helps with luggage, documentation and wheelchair to a drop-off area inside TJ airport then they hand off to someone from the TJ airport. It was $60 round trip, and I gave cash tips but that's up to you.
