Go to apple.com, select support on the upper right menu, then you pick iPhone, repair options, then "Make an appointment or request service". After that step you select the problem (screen, battery,...) then "bring in for repair", insert your Apple ID and password then select the store you want to make an appointment. You can choose date and time. Usually there's a line schedule the same time as yours so expect a little delay.
