If you want the "real id" take two proof of address, I.e. utility Bill's or bank statements that are made out to you at your present address. The 'real id' allows a wider US means of ID.
It depends. For most people, some proof of who you are such as a birth certificate or old I'd and proof of address such as a bill. Junk mail, personal letters, etc are not usually accepted as proof of address
If you're a woman, you have to have certified proof of all name changes. So if 2 marriages, you have to have your ss card, certified birth certificate, certified marriage certificate, certified divorce decree & then new marriage certificate. Etc for every new marriage. In addition to the 2 proof of addresses & your old unexpired liscense
