If you are renewing a license you may be able to do it Online without an appointment. If you want to get a MA real ID you need to make an appointment online. If you want to speak with someone at the RMV on the phone be prepared to wait on hold for 50 minutes. Appointments cannot be made over the phone.

Yes you do need an appointment for your ID the only thing you don't need an appointment for is registration

I believe you could do both but I would definitely set an appointment just to be sure there is able to ask you if you have an appointment in front of the building the officers will ask.
