If you still live in the area, go to the health Dpt and they will give you a blue paper to fill out "application for florida birth record" it will say. Its $13 and $7 for each additional copy if you want extra. You can get a protective plastic sleeve if you choose also, for an extra 3 bucks! Hope this helps! 😆
