I really need my medical records but I can't get anyone to follow through on emailing me the paper to sign for their release. What am I supposed to do now???

You can make your own medical release form with your name, date of birth and signature stating you want all your records on a plain page.

I had to go in and fill out form and if it is just your medical records and no films or scan results you can wait for them and get right then.
