Go online and request an appointment. I am currently being treated at Moffit after being treated at Loyola Hospital in Chicago (a highly rated hospital for cancer) I am amazed how much better Moffit is. As for being rated number 5 in the country, I was told not to go to the higher rated hospitals by several people I know and trust in the health field because there you are more of a number. At Mottit they really care about you. I travelled from Chicago to Tampa for my radiation treatment back in May. I'm so glad I did. Another thing to keep in mind, if you are under going treatment at Moffit you may be able to stay at Hope Lodge run by the American Cancer Society for free. Wonderful experience being around other people going through the same thing as you. Made many friends while there which I have continued to contact after leaving Hope Lodge. Both the patient and one caregiver can stay at Hope Lodge. Check early for requirements as there is usually a wait list.
Call Moffit and also have the doctor to call there they are great doctors there
I would have your dad's oncologist in Iowa contact the Head & Neck department @ Moffitt to see if a transfer of care if possible. Besides that I don't know much about the process. Hope that helps.
I too have stage 4 melanoma. I've had it in my brain and left leg. I've been going to Moffitt for over a year and a half and would highly recommend them. They are about the 5th best for cancer treatment in the US. Contact them and they have a great support staff that will help you with getting everything going support wise on insurance or Medicare, etc.
