Yes, the bathrooms are accessible as well as the rest of the restaurant. It's not perfect, the restaurant has some narrow areas that are passable but will be trickier to maneuver through. But the bathrooms have a good amount of space to move around in and they are individual rooms instead of stalls.
Not sure but if the entrance is then the bathroom might be or at least level with the rest of the restaurant

Yes its all one level and the front door is on ground level without stairs or obstruction. Tables have plenty of space for someone in a wheelchair.
