It's at the north end of Bernardo Center drive. In the same parking lot as Souplantation and Stir Fresh.
It is kind of hidden and the street is not labeled on a street sign but instead on a commercial center sign. I have seen several Uber drivers miss it. There is a souplantation in there as well.
