Once a year there is a $40 charge on top of the monthly fees.

Yes, annual fee is every September for both Black Card and White Card members. The monthly fees are withdrawn monthly from a checking account, so you have to provide the bank routing number and your account number when you sign up.

I worked there a couple years ago the way it works is, you will need to use your bank account info aka your routing and account numbers, the 50$ dollars your referring to is the "annual fee" it's a once a year service fee for new equipment or club repairs.
