Reach out to the customer service on phone or at I believe any costo store. I'm sure they'll be able to get you a new one based on your name and address details.
Go to the customer service desk and they will replace it
Just go to customer service and tell them, you still have an account so no need to reapply, they will just make a replacement for you once you let them know. Id probably call before i go in so there is no confusion.
