In order to expedite your request, print out the attached and complete the "Authorization to Use and Disclose Health Information Form" (web page with download link below). This request can also be faxed (909.920.4745) or mailed to the Health Information Management Department for processing. https://www.sarh.org/patients_and_visitors/patient_records/ Copy of Your X-Rays If you would like a copy of your X-rays, please contact our Radiology Department at 909.920.4710. X-rays are considered part of the patient's private medical record. To obtain a copy of your X-rays you must go to the Radiology Department in the main hospital. We have a medical records office open m-f normal hours. I would call the operator first and have them transfer your call/question to the office before coming in
