I’ve seen a few guys wearing cargo shorts, Birkenstocks, & Hawaiian shirts there (along w/ jeans of course); but most people wear some variation of casual chic Or business casual: A suit, dress, pretty skirt, slacks with a jacket (you don’t see many ties, though...).

No not for the upstairs dining room which is the one I go to now. Years ago I went to the more upscale dining room downstairs and that a higher standard.

No, but in my experience, they will give you snooty, sub par service if you're not dressed to their expectations.
