U don't need to email them if u r signed up for www.myhealthevets.va.gov then u can just download what u need. U do however need to have it verified in order to use message portion of it. Maybe u can have someone bring the form to u to sign and then back to the hospital library for verification. Sign up online its easy just follow the steps.
I do not believe there is one as fax is used to date. Call 813.972.2000 so the operator may direct you to records for further details.
