They have a place to take the kids to be measured and they give them wristbands that are color coded so you know which rides they can ride before waiting in line. Makes it so much easier!
Several carnival type rides for very small children, otherwise i believe it is 42" accompanied with 16 yo+ or 48" un-accompanied.
It is based on height. The measurements are either 36 inches or 39 (Can't remember exactly which one) and some are even as high as 42 inches. They will not budge on it either. If your child does measure up in height they will still require someone over 16 to ride with them.

Yes, based on height, but there are rides for kids of nearly all ages. There is a section that has rides for the tiny ones. My kids loved them growing up, and now my grandkids do!
Depends on the area of the park. There is an area (I believe it is the county fair area) where there are several small children rides.
