You need a card both to get in and purchase items. If someone buys you a gift card, you are allowed to spend it without being a member.
You CAN get in the door without a card.
Technically you can probably get in without one, but to make any purchases you will definitely need one, or be with someone that has one and they have to pay.

Yes you do need a membership but if you buy lots of groceries or have a big family it is well worth the cost of the membership to go to Costco
You need to at least be with a person who has membership and they have to purchase. But, you can give them cash or your card and they can do separate transactions. The only time have to use members credit card is if buying alcohol, at least that's true in Oregon and Washington.
You only purchase prescriptions and hot dogs and similar snacks.
