To save a lot of time and hassle, your best bet is to call the Oregon Membership Service number. Have your membership number ready and they'll get you all set up.

They have membership services at the north Lancaster medical clinic at 2400 Lancaster Dr. and the Skyline medical clinic you can sign up there.
