Actually, if you husband currently carries a valid Oregon credential, he does not need to complete form 265pip - only 265. Doctor needs to complete authorization. Term length and doctor approval valid during length of credential and requires renewal. Oregon does not have handicap license plates.
He needs to fill out 1. Parking identification card and disability parking permit application (FORM 735-265PIP) to apply for the parking ID card and permit at the same time.
