go to the d.o.l. website and schedule an appt(most likely 30 days, you can't use the voucher online, u can get one online but it will either be 24 or 54 for a replacent or lost) then call the 800 number if the office is still covid closed and they will mail u one out then bring that to the d. o. l. in person appt u made
Call and ask if u have to make an appointment or if you can walk in, and ask what you need to do for the voucher
