Bad knee. I wouldn't recommend it. Steps are uneven.
If the knee is in a compression cast or not very bendable, the tour would be difficult. There are hand rails in many places to help going up and down the steep stairs.

Some of the passageways are narrow. A few of the stairs are steep. And at least 1 short cave that involves stooping low.
I did it with my 2 year old, 4 year old, and a bad back. It is very fun and the tour guide was very informative
It isn't too bad. I have a torn rotator cuff and a bad knee from an accident and I was able to do it. I was sore the next day, but it is mostly going down stairs. I would say you only climb up about 20 during the whole experience and most of that is at the end to get to the elevator that takes you back up.
