These people are very reasonable I think my renewal was 70$, but to start ur license it a little more somewhere between 90$-120$ that pays for the doctor to do his job. Then the lovely lady's up front will put all ur paper in a envelope and now u put a additional 60$ for the license to the state and if u have a caregiver it's another 25$ for him in with ur paper work. They get you the paper work and show you what to fill out. Very easy process all you have to do is put it in the envelope and mail it!
Hello! We charge $90 with medical records, $125 without medical records. It would be considered a first time application if your card has expired. 125 w/o records, 90 with. It's the paper form, you have to file. You pay the state fee as well
They have specials for two people, with or without medical reports. A single person with med records is $99... without med reports it costs $149 (they said $139 but l paid the $149)
