Not officially, but I wss there for Sunday tea and the majority of the women were in gorgeous Sunday dresses and nice heels.
My wife and I usually go once a month and we always dress Business Casual.

It's a place for a special occasion, dress the part.
You can get away with jeans and long sleeve shirt, but may feel out of place. I suggest businesses casual.

Yes, its posted at this hostess station. They say jeans and shorts are welcome upstairs in the bar, but a slightly more formal code should be followed in the restaurant levels. As in, long pants, shirts with sleeves, etc. I wore jeans and a sweater to the bar. I felt a tiny bit under dressed but not in any noticeable way.
Business casual in the dining room. I'm not sure about the bar.
Not that I'm aware of, I've been there plenty of times and see people dressed all kind of ways. Although it is a little upscale.
