Just need your birth certificate, social security card and proof of address
If you dont have a social security card though you can supply your medical record with your number on it and address it'll cover proof of your number plus address
If they were suspended you can't get license in another state. You'll need to contact your state where you were originally licensed and check on reinstating. If that's not what you're talking about, I would say just call the Bmv and ask because I'm not sure.
