You definitely need a card. Verification for a doctor cost me 149 for the year. Shop around and see. Once verified, you register on the NY State of Health website and you get your temp card to print out. I recommend printing it out right away or saving the pdf and emailing it to you. Verilife has no problems printing out your temp card for you as well but you must email them first.
