Start at the state website, see if you have a qualifying health reason
Simply having surgery won't qualify u in this state u have to be in long term chronic pain and have documentation from drs to prove it, but if u actually qualify the instructions are easy to follow and easy to find on google, starts with signing up with the state then u see the dr, about $200 cash only then u pay the state the $50 and wait a few weeks and it will arrive in the mail
