If 0 stars were an option, that’s what I’d choose. My grandparents were there and there were so many things done incorrectly it’s astounding. No one knew both grandparents were fall risks, no one knew grandpa was a choke risk, the call light to their room didn’t work the first couple of days, they didn’t give my grandpa his evening or night medications the first night, they didn’t put water in grandpa’s CPAP so his nose bled all the next day before anyone looked at it, they went hours without water when they first arrived- the list could go on. Run far, far away from this place. I’m surprised the State Health Department doesn’t just live there because this facility should have a million complaints and tags.

Went to visit a loved one and I am thankful we got him out of there. This was his room. Blinds and walls covered in I don't know what nasty stuff. Bathroom was a mess and nasty. He had no TV to watch so just had to lay there looking at this. This place is so sad and filthy.

Poor service provider and they are very mean and rude to the patients....i have seen this done where a lady cried and has bed sores and poor personal care and this is not right...

Recently had a family member in need of rehab here at greenfield. Greenfield was very accommodating to my family members needs especially the DON, Tara who was very hands on and reassuring during this process. The admissions team was very kind and helpful from transition to the hospital to being available anytime we needed them. Highly recommend greenfield in the future for anyone ‘s rehab needs
