Yes, but every scar is a case-by-case basis & scars are required to be healed for a few years before they can be tattooed over.

For me, I presented a design and set an appointment for a week later. During that time, he traced it up, slapped it on my leg when I got there to see if I liked the position and size, and tatted away. So if you're asking what I think you're asking, they follow a design they place on your skin with purple ink as a guide. Don't know of any places that do it any differently.
