I believe the discounts are for the state insurance AHCCS and possibly Medicare. Some private insurance providers will carry a part of the cost, but it could be a per-case basis. Best thing would be to call them and arrange a consultation :) hope that helps!
Yes you can at all the dispensaries but with limitations, additional 16% tax, and with little to no protections when it comes to police, CPS, work or housing.
The doctor is so nice and the process is smooth like butter. You'll just need your licence(Arizona issued I.d.) and medical records. The medical records are proof of your reason for being there. If you have ebt(food stamps) you'll get 50% off of the state fee which is $150, bringing it down to 75. You just need your approval letter stating you recive benefits. $90 doctor visit as well. So $240 w/o EBT or $165 with EBT.
We currently charge $90 for a certification, plus the AZDHS application fee of $150, or $75 if you have proof of current Food Stamps. This price is for those patients who have current medical records from within the past 12 months. If you haven't been to a doctor in the past 12 months and your condition is within the scope of Chiropractic, we can do an up-to-date exam for a current record for an additional $45. Total price is $240 with records or $285 without current records.
If you do not have your medical records, but you have seen a doctor in the last 12 months for a qualifying condition, we can obtain those records for you. If you haven't been to a doctor in the past 12 months for any qualifying condition, but have muscular/skeletal severe and chronic pain, our physician can perform an up to date exam in relation to that pain for an additional $45.
