Hello and thanks for your interest. We do not quote fees via email or over phone, as we would need to see you first for an exam and x-rays and go from there. The fee for that is $145.00. We are not a contracted provider with your insurance but as long as it’s a PPO we can file a claim for you and then you are reimbursed directly by insurance.
