The best experience I've ever had dealing with a government entity. There was a very short wait and the people doing security are wonderful. The person that help me with my application process was fast friendly and so efficient! I was there 45 minutes that included time to drive there! Thank you for making my visit there so painless and pleasant!

Yes
You will also need her death certificate
You provide two other forms of acceptable identification, the information is on their website.
I don't know the PERA answer, but you can draw against your ex's SS if you didn't remarry.
Hi David, Just wanted to let you know that the information you posted is public. This doesn't actually go to the Social Security office. It's a page on Google that offers reviews and questions you can ask the general public. I just wanted to tell you in case you want to remove it as it has your personal information on it and anyone can see it. Have a great day.
A lot has resolved in three weeks sense I posted this question.

Pretty sure you have to have the routing number and stuff for the other bank account and then you just take all that in but I would suggest doing it off their website if possible it's a lot easier and a lot faster
Thank you for the information and help
You will have to apply for a new one.
You need to set an appointment I believe.
Go to the US Social Security website and explain your situation to them they should be able to guide you on the steps you need to take to replace the card.
You should be able to call and get a copy of it, the SSA also regularly updates their website with changes to the amount with the rise and fall of cost of living.
Avoid go at all costs! Instead, first try doing it over the phone! Instead of going there and having to deal with the security guards who will make u literally strip down (belt, shoes, the works), and then just sitting around for hours. It's VERY FRUSTRATING T0 SAY THE LEAST!!
Yes
Online account or walk in to get a ticket and they'll change it for you
Open 9-4 Monday, Tuesday, Thursday, Friday 9-12 on Wednesday
Go to the Social Security offices I would go in the morning when they open
No, Medicare. I spoke with SS office and determined all is good.
Just change at th3 post offices. Then go upstairs and do the same
I would just walk in
Call your local Social Security office. There is in in Greeley
No fee
Hate to tell you but its near impossible to get a live person. Easier to go to the office, go early in the morning for least amount of waiting time.
Yes, you'll just wait longer to be seen.
Perfectly good drain on the economy.
Go in to the local office with death certificate.
You can call ahead to schedule an appointment. Or you can walk in. But the wait can be up to an hour before someone sees you. Just depends on the day. Open 9-4 but closes at 12 on Wednesdays. Not open weekends
I'm not sure if they are legitimate, but I'm fairly certain they are not a government nor social security committee. Donating to that cause is optional and, though their efforts may or may not influence how the government rules on social security and medicare, I'm pretty sure that being associated with, or donating to them, or not, has nothing to do with whether you will receive social security benefits.
You go to where you get your Vehicle plate they require a form from your doctor
I think you'll need his or her birth certificate
