Yes, dogs are allowed in the outdoor seating area and the front porch but not in the dining room.
I can't answer the first question but the website says lunch runs until 430pm. Enjoy!
Reservations are not necessary, but they only have 16 tables so if I were you, I'd make a reservation if you don't want to wait as a walk-in.

Usually. Sometimes they run out of ingredients like any restaurant or run out of a special, but they are pretty awesome at accommodating patrons and even putting in orders that AREN'T officially on the menu.
