Plan to arrive 10 minutes before your appointment. If you need to fill out paperwork at the office, please arrive 30 minutes ahead of your appointment. You can download forms on our website under the “Patient Center” section to save you time.

We ask that new patients bring the following to their first appointment: (1) completed patient forms (found on our website); (2) name, address, and phone number of your pharmacy; (3) insurance card(s); (4) driver’s license; and (5) any previous medical records, especially test results and x-rays (if needed).
