This Health Department is in need of major training rehabilitation. I received my 1st Monkey pox vaccination 30 days ago. The nurse that injected me was incompetent & wrongly placed the syringe in my arm causing half of the vaccine to run down my arm. I was highly upset & don’t feel that I have been properly vaccinated. I reported this to the acting supervisor who apologized & told me that she would personally give me my 2nd vaccination. Today was suppose to be my 2nd vaccination but when I called to verify the appointment no one answered the phone. A recording stated that all the facilities are closed for training. So why didn’t anyone call me to change my appointment time which IMO reflects more incompetence🤷🏽♂️. This facility is not at all customer service friendly. As of now I plan to make an appointment elsewhere to receive my second shot. I don’t trust this Health Department nor am I going to allow them to gamble with my life. They’ve shown me twice now that they are incompetent. I’ll travel to Atlanta to receive my 2nd shot before putting my life back into this Health Department hands.

Every single time I have an appointment they cancel it and I have to wait at Lear another 6 months to be seen. This is the worse so supposed personal office I’ve ever seen in my life. They sick and do for Columbus. You can’t even get your teeth fixed in this broken as place. In one year they have rescheduled 3 appointments, it’s like everyone it’s time for me to be seen they call and say not to come, and then the last at the front det will have the nerve to tell me I missed my appointment. I have NEVER seen anything like it. I don’t know who the owner is, or hiring person. But some people need to be fired. And the list at the front fest has a horrible attitude. Not professional at all. If you can help it don’t waste your time with these people.

I have to give one star, because the security made me turn around and leave to get a mask, I had to drive 16 extra minutes, to come back and the services I need is not at that location. What a waste of everybody time. Not only that, they are not pleasant. They are mean as hell, a hello would be nice. How are you? Sometime. Then when I ask about my service, they acted as if I was wrong. Thank you security for being sooo rude and unhelpful Updated: I returned the 3rd time after being told my services was not at that location, to find out ALL the services I needed was in fact at that veterans location. 3 times in one day!

There's no appointment needed to pick up a birth certificate from the health department. She will need a state ID or driver's license if she's 18+, if not, her parent/guardian will need state issued identification and to visit the new location on North Veterans.

You have to go get your birth parent and your birth parent has to have ID and social then you go get your social security card from social security THEN you can pay 25$ for your birth certificate
