This place is so great! The staff is so wonderful. They make you feel at home and are happy to assist howeve they can to support the patient is putting their health first. Onlg downfall is you might not alwyas see a Dr. However they have nurs practitioners that are super!

in order to be seen by the clinic, you must bring in 1) a current ID, 2) two months most recent pay stubs for everyone in household, 3) food stamp award letter including current name, address, and benefit amount (can be found easily at MyDSS.mo.gov, 4) documentation of elegibility of federal, state, or other income benefit assistance including SSi, SSD, VA, and/or workers comp. 5) some applicants may be asked to supply a recent medicaid denial letter, verification of shelter, a notarized letter of support, and/or a letter of health insurance termination.
Also proof of income of everyone in the household for the last 2 months

Come to the clinic and fill out new patient paperwork. Once you qualify by income guidelines, you can be scheduled for an appointment.
