Hi Agata, We charge $90 for a renewal. You can schedule online via our website, or you can walk-in during business hours, usually no longer than a 10 min wait.

Chronic abdominal pain from Celiac's would be the best condition to be qualified under. If you have medical records showing a celiac's diagnosis, please bring that with you. We can also request records. You won't need records to get the license, but we will need them in the file before you renew again next year (per new State rules).
For $75 the clinic will handle the online application process following the Dr's recommendation. Most patients do this on their own by setting up an account with the state licensing agency, but some patients prefer that we do it for them.
