They are the worst nursing care facility and they need to be closed down for good. On a scale of 1-10 I would say a -1/10. The staff have to worst attitudes ever and the management is trash and everybody needs to be fired immediately.

Aug 2 - Aug 26, 2022 - Short-term After Mom having a month long stay at Garden City Hospital and a horrible incident that left her with a broken arm and barely able to walk, Cancer, and Renal Failure, I was given 24 hours to find a rehab facility. TAKE A TOUR! There was more staff here on a Sunday than at Garden City Hospital on a Monday. ***Side note*** These people are doing a very hard job I would never be able to do, if they need multiple breaks throughout the day to refocus or break the monotony I am 100% ok with that. They need to keep their sanity and mental health up to par to keep giving great care to our family members. They probably have a breakroom because I NEVER saw anyone sitting around. Food - Just as any "institution" type facility you get what you get. They have to accommodate so many different dietary needs it's going to be flavorless no matter where you go. Four Chaplains WILL allow you to bring food in for your family member or you can always Door Dash from their favorite restaurant. Physical Therapy - LOVE THEM!! Tony and Jenny were my mom's therapists. They have awesome personalities and are very upbeat, positive, and genuine. They would pop in to check on my mom and have light conversation regardless if it were her turn or not. They kept her feeling human when it wasn't physical therapy time. I loved that. Social Worker Angela – She’s an Angel (with an extra “a”). She is a great resource to have on your side when it is time for your family member to discharge, ask questions, be honest with what your family member’s needs are when they discharge to make that transition easy for everyone. Transportation - This was a big deal! I am so happy they accommodated my mom with getting her to all her appointments and the short notice changes. This took one less stress off my plate by getting her to Dialysis 3 days a week on time. Nursing Staff - AMAZING! I cannot say enough about these women and men! I don't care about the food, I don't care about the visual aesthetics. I care about how she is treated and they are all wonderful. I couldn't ask for a better team taking care of my mom. Mom's main Day Nurse was Irene...... I would come in all spastic because outside of Four Chaplains I have 100 things being thrown at me at once regarding my mom and I have no help. Irene is like this calm, cool, collected woman. Just her presence is calming and it made me calm. Days I would be breaking down Irene, Christina and the other ladies took notice and gave me pep talks I needed, words of encouragement, or just a hug. They got me refocused so I could go back out and tackle more of what was being thrown at me. There are no words to describe how thankful I am for all of you! Activities - Mom was a non-participant, however! There is a calendar of activities posted in each room for social enrichment. These ladies are fantastic! All-in-all we had a great experience. Would I have mom go back again, YES! Am I curious as to how other facilities are run, NOPE! Keep open lines of communication, do not make them guess what makes your family member comfortable, every person is different and has different needs. My priority and main concern was Mom's overall treatment, physically and mentally. I could not have asked for a better place and the staff was top notch. Food, meh, but we are allowed to bring in food. Facility is nothing fancy, patient hallways are clean and decorated in calming grays, individual rooms maybe a little outdated, but they keep it very clean. Important Note! -Many facilities use bed restraints, Four Chaplains DOES NOT. -Some facilities force the patient to wear adult diapers with rubber pants from admission to discharge, Four Chaplains DOES NOT. These are available if your family member is incontinent, they keep them in each personal wardrobe closet so it is discreet and private.

I loved the physical therapist all of them they were very nice and very encouraging but as far as the facility the nurses the director of nurses the director of the place very rude at times especially when they had told me that I had a urinary infection strand that was contagious they made me feel very dirty and that I was contagious and told my friends that were there to visit me not to touch me that I could cause them to get sick. It turned out I didn't have the 2 urine infections because the hospital gave me another urine Coulter and they were both negative so I also was taken to the Hospital by an ambulance and stayed at the hospital for 2 days and isolation because that was what I was told that I had in the hospital was told that I have that they also gave me the wrong person's antibiotic IV antibiotic, one of the first weeks I was there. and one time they they tried to give me a pill that I didn't need she didn't know the nurse know what it was for she said at first it was for a blood pressure then she said it was for a UTI which I didn't even have a UTI. I will never go back there again. Or even send family or friends there.very disappointed. I loved the physical therapist, Brad ,Natalie, Tony.

PURE NEGLECT!! My Grandma was there for 10 days. When she returned home, the NIGHTMARE she experienced was revealed. Her teeth hadn't been brushed, her tongue was swollen to the point she was unable to eat or drink and She has a severely infected and open wond from sitting soiled for a prolonged period of time. You call that caring for our loved one? There is no circumstance, to justify why this would happen under the care of medical professionals. The staff didn't notify anyone of any changes that could indicate she had declined, wasn't doing well. My Grandma (96 yrs old) has lived through a lot of things in this world. I cant imagine how depressed and unworthy she must have felt. Depriving her of basic human decency! Its taken four days for her to decompress enough to be responsive. She still has been unable to eat a hot or solid meal. Your doors should be closed and your licenses revoked!

Does Four Chaplains provide hospice care ?

They do have hospice patients there, but I believe that you have to have an outside hospice agency, like Angela Hospice or Hospice of Michigan, to be in charge of the orders and then the nursing home staff carry out the orders.
