Oh my gosh, they are so rude!! Every single time Ive talked to someone there its been a negative experience. I was approved for food in april and its june 16 and still no card! I dont know whats going on but cmon this is outrageous. Sad to pay taxes and plenty of them to be dealing with injury and surgery and needing help to be shafted like this...

Yes, they have two. One outside directly by the front door and one inside the lobby.
Take documents from WV and identification to Ohio and they will tell you what further documents they will need.
