No communication between departments food not delivered on time or hot food not held when out of building for dialysis and the man in kitchen can be very nasty and when on Special diet they serve food that is not on diet and seams that medications not given on time I’m writing this review on behalf of my mother in law who is a patient there

This small facility is one of the better rehab facilities in the area. It’s beautiful and cozy. The management staff are attentive and willing to go that extra mile. I recommend this facility to anyone looking for short term rehab or long term care

My Father is in the rehab section. He has waited 4 hours to have his soiled diaper changed. I don't know about you, but if your baby had a dirty diaper would you let them sit in it for 4 hours. He has waited 2 1/2 hours to have his trach suctioned. Do you know what happens if your trach is clogged? You suffocate. His reading glasses have disappeared and a brand new pair of sneakers that he got for Christmas and only had them on his feet twice. Tried calling and emailing the director of nursing, the assistant director of nursing and the administrator. Well do not use the phone numbers or the names from Mystic Meadows website because all three of them mentioned above are no longer with the facility. Katie the speech therapist is compassionate and caring and the physical therapy girls are all so kind. Nurse Debra was the only nice nurse I have met so far. This facility needs to reorganize.
