First was in 1889, second in 2008. It was moved from what would become 143rd and Convent Ave to 141st and Convent. Then it was moved to St. Nicholas Park by NPS.

I believe it's a free self guided tour. Check their hours of operation, they are not open every day.

Yes, the bottom floor is open to the public and the top floor is open as long as you're escorted by a member of the staff.
That is the closest station. The 135th Street Station also works but it is near CUNY, also worth a visit for the Gothic Revival architecture. But 145th is closer on the A, B, C and D lines.
