A family member needs a physical for Preop clearance. Do they do this @ this place? Also needs the form given by the surgeon to b signed and faxed.

Yes they do that @ Concentra and also Patient First to my understanding does this or they can go to their Primary Care Doctor to get a Physical for Pre-OP.
