There is a man who fell in the group home 2nd to do so on the urinated on uncleaned floor in the bathroom. He came back after paramedics took him away a couple weeks ago yesterday night. And he is wearing a neck brace now also in a wheel chair and you are telling me there was and is no problem. Shame on you all because this could have been avoided had you all acted and done your job when I suggested. And when I tell this man what has been going on in my life that I reported things in this home and his falling was no accident it was just laziness and lack of work ethic. You need to act now with this place at 1469 East Ivanhoe St Gilbert Az it's a danger ⚡ I also have not gotten medication in three days for pain medicine an I am now in the hospital severely suffering. A lady from the Dept. of Public Health called me and left a cryptic message about "lab results" from recent routine blood tests I'd had at my doctor's office. This was concerning, so I called my doctor to see what the problem could possibly be––there was NO problem. The department never should have contacted me. Very rude to scare a pregnant woman like that. To top it off, the lady never returned my subsequent phone calls to conclude the matter.

Went to the clinic and they turned 90% of people away. No walk ins...just appointments. They decided it last Monday and announced it online. They did announce no appointments were necessary. All of us are angry. P.S. they had two professionally trained guards which denoted they expected commotion which was suspicious. Who did they allocate injections to? Those few with appointments looked professional and well dressed. Don't believe their comments below.

The inspector was nice and did do an inspection but he clearly did not actually care that all restaurants have all of their managers food safety certified. This is very concerning and I wish they'd do better. Food safety is a serious issue and all managers are supposed to be in some shape or form food safety certified. I am disappointed in the Maricopa County Department of Public Health and Safety. The inspection resulted in the district manager being present at the time of the inspection so that's all they cared about since the district manager is food safety certified. They didn't even ask or check the other managers that actually work at the store. I would not eat at Wendy's off of Gilbert and Baseline as they do not have any actual food safety certified managers.

Hi, we do not handle or issue food handlers cards. Environmental Services Dept used to do this, but not any more, and the cards are certificates now. All trainings are now done through third party vendors which should let you print your certificate. If you took it online, you would need to contact that vendor to get a copy of your certificate. If not, you may need to take the test again through one of the vendors listed on Environmental's website (enter URL without spaces) maricopa . gov /5114/ Certificates are only good for 3 years, so if you had a card, it would not be valid anymore.
