You only need one physician certification but you need to bring in documentation from your primary care physician documenting your qualifying condition

Yes with a physicians diagnosis you must have that when you come in and a Colo ID
Chronic pain and PTSD are qualifying conditions being under age you would need to bring an extensive documentation for both those conditions you would need to see two doctors and we only have one so you'll have to get another doctor to sign off on your conditions
