Yes, you will need a permit
Contact the building department to see if you have any open permits. Have your address, section, block and lot numbers available. Section, lot and block can be found on your survey or sometimes real estate tax bill. All work filed should have a completion certificate.

It depends the deck would have to be looked at and determine certain building code requirements.
No; but there has to be a landing at least as wide as the door opening and at least 30" deep and not More than 8" lower than the threshold. Then to reach ground level each step down has to be no more than 8" lower.
