From Nassau county, I would get on the LIE going east and get off at Exit for route 112. Take a left heading north. Make a right at the gas station and then left onto Middle Island Rd. The facility is on the left. If you get to Granny Rd, you went too far, but you can make a right onto Granny and a right onto Middle Island Rd and right into facility
You should contact the puppy program and they will give you a new one.
