I went here to get my baby's birth certificate. The ladies were fast and efficient. I was thinking it was going to take a while, but I was pleasantly surprised and waited maybe ten minutes tops.

Today I was at the Department of Health. I kept calling and they never answered the phone and went I went there to a vaccination they were super rude. I walked into a mess and loud room. They told me what to do but with extra sass. I was waiting for an hour and then I asked again what was taking so long. Again they said you can continue waiting an hour and a half more because everyone is on their lunch break. Never coming back. We still needed our vaccination, went to Provo and they did it the right way. Provo on the other hand, is way better. Provo fixed everything American Fork couldn’t do and they were super friendly. Provo did everything under 20 minutes while American Fork rudely told us to wait longer. Choose wisely.

Have had every so often the milk from Walmart go bad way before the expiration date! I take it back so they will know! What to do to get this fixed don't known.
Hi Carla, we do not do COVID testing at this location. However, we do provide COVID vaccinations on certain days. You can view these days and times here HealthEvents.UtahCounty.Gov
Go to the closest health department, fill out a "release of information" form, bring your photo ID and they can look up the record on the State web-site, pending on if the pharmacy reported it to the State.

Joi, I got amended birth certificate a few days ago. You do not need an appointment at the American Fork location.
It probably depends on what your getting. When I got my food handlers card, all I needed was my drivers license, my printable temporary food license, and the money. I don't think more than ID and ssn (or reason for exemption) will be necessary, but you may want to look up requirements for what you specifically are doing there.
