Birth certificate
You will need a statement from your doctor/form stating that you need it.

Only by apptointment u have to call them to get one and then u can go in at that time other than that the doors are closed
9 to 4 on M, T, Th, Fri, Wednesday 9- 12 ,Sat and Sun closed
It can be a copy of your ID
If I'm not wrong, I think you have to come in to get it. Bring birth certificate and any necessary ID cards. Check their online website to see if there is any other info that may help you.
If the 1st or the 3rd fall on a weekend. Check will be out the Friday before.
Go online and print off a copy...you may have to setup an account if you haven't already
I just had to get one for my daughter last minth and thwy would not let me use hwr birth certificate i had to guve my driver's licence if u are not a minor u will hace to use yours or your id
URGENT CARE DOCTORS USUALLY DO...
it will not be cut off. If you do it to many times then they may red flag it
