The walkway entrance from the Brooklyn side is at Meeker Ave and Van Dam St. From the Queens side the entrance is at Laurel Hill Blvd and 54th Rd. There is street parking nearby on either side.

Old Kosciusko bridge was opened on August 23rd, 1939.

No, the old Bridge was brought down and I believe it was used for scrap metal. They are still doing construction today as for what I see will be a second Bridge that will be along side the new Bridge they already put up.
I do believe they kept the original plaque.
They already have put up new light beams on the new Kosciuszko Bridge and I believe they will do the same on the newer Bridge that's going up as well.

They won't but I do believe they are adding another part to the construction of the new Bridge. You also have asked this same question before.
The Old Bridge was dismantled. The new Bridge is open. I know they are still doing more construction these days and it looks as if they are building another part to the Bridge that may connect from the LIE to the BQE apart from the Kosciuszko Bridge.

On the Queens side, the closest public transportation would be the bus, q67 to the 43st/54av stop then walk 5 blocks. On the Brooklyn side it would be the b48 to meeker av/stewart Av stop. Its important to note there is no pedestrian walkway at this bridge as of today but there will be one in the future.
