All paperwork that was given to you by your physician’s office. Two forms of ID (one must be a picture ID). A list of ALL medications you take, including vitamins or supplements. A book, magazines, music, etc and please leave any valuables at home.
Payment is due in full at the time of service. We do not offer any payment plans.
