I haven't been here as of yet, I just have a question, what's the procedure of obtaining a card ? I've been diagnosed with uterine fibroids and because of that I suffer from extreme chronic pain from period cramps and it also causes me to have EXTREMELY bad back pain. I really need this card !

Chronic pain means no pprwork from doc
So the total fees can be from $175 to $300 depending on how many discounts you are eligible for.
Hi The az department of health requires you conduct an in-person physical exam with a doctor every year (every other year starting next year) in order to renew your card.
